Initial Consultation
Oral Surgery Consultation
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. In some cases surgery can be done the same day as the consultation. However, we may decide to schedule a second appointment for your surgery if you have a complex medical history or an extensive surgical treatment plan.
Please assist us by completing your registration form and by providing the following information at the time of your consultation:
- Your surgical referral slip, and any X-rays if applicable
- A list of medications you are presently taking
- Your medical and dental insurance information
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian.
If you are planning on an IV anesthetic, please have nothing to eat or drink 6 hours prior to your appointment on the day of surgery. You will also need an adult to drive you home.
Please alert the office if you have a medical condition that may be of concern prior to surgery (e.g. diabetes, high blood pressure, artificial heart valves, prosthetic joint replacements, rheumatic fever, etc.) or if you are on any medication (e.g. heart medications, aspirin, anticoagulant therapy, etc).
X-Ray
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time for delivery from your dentist or physician, please pick the x-rays up and bring them to our office on the day of your appointment. If additional films are necessary, they can be taken at our facility.
